2010 Call for Proposals/Licenser Worksheet

If you are a confirmed speaker for the 5th Annual HIV and Substance Abuse Prevention Conference in Corpus Christi in September 2010 or the National Harm Reduction Coalition Conference in Austin in November of 2010 please complete the following form. 

The following worksheet must be completed by all confirmed presenters/instructors for the purpose of satisfying Licensing and Certification Standards. This form will serve as documentation for both the presenter and the Texas HIV Connection, approved by the Texas Certification Board of Addiction Professionals, the Texas Board of Social Work Examiners, and the Texas Board of Examiners of Professional Counselors as an Education Provider.
